What we collect, and what we do not collect
We take privacy seriously. When you use RootLine, the form asks for contact details and general request details only so we can help with matching.
We do not ask for a medical or dental history, medications, conditions, or sensitive records on the site form. Please do not send private health records through RootLine unless a licensed provider directly asks you for them through their own process.
A simple request may include:
1. Your name and contact information
2. Your location or preferred area
3. Language preference
4. General scheduling or service interest
The licensed provider you choose may later ask you for health information as part of their own intake. That is separate from RootLine.
No guaranteed results, and what to do in urgent situations
Dental care can help many people, but no treatment result can be guaranteed. We do not promise success rates, healing times, pain levels, or long-term outcomes. What is appropriate depends on the findings from your exam, your goals, and the provider's judgment.
